NJPRA Consumer Scholarship Application

Through the generosity of the New Jersey Division of Mental Health Services, NJPRA is able to offer consumer scholarships to attend for one day of the 2008 NJPRA conference.

The conference dates are Thursday, November 13 and Friday, November 14 at the Woodbridge Hilton in Iselin, NJ.

Please read the following conditions before applying!

Scholarship Eligibility Criteria:

1. Must be an NJRPA member in good standing at the time of the application and conference.  Therefore, in order to apply, you need to either join NJPRA for the first time, or renew your yearly membership for $10 (if it expires prior to the conference).  You must submit the fee with this form, if applicable.  If it is not attached or you are currently not a member, your application will not be processed until it is received.

2. Scholarships are intended for consumers of psychiatric rehabilitation services who have not other financial resources to pay the registration fee. 

3. The scholarship selection process is competitive (i.e. applicants are not guaranteed a scholarship). The selection committee is looking for complete and thoughtful answers to the questions on the application. 

4. You must be available to attend the conference.  Scholarships are only granted through the application process and cannot be transferred to another person.

5. It is necessary for the scholarship committee to be able to reach you to award the scholarship and confirm reservations for the conference day.  Please leave a day and evening phone number where you can be reached.  The committee prefers to speak with the applicant directly.

6. Scholarship applications must be received by Friday, October 31, 2008.
